The most useful supplement list for a woman in her 60s isn’t built from a marketing catalog. It’s built from data on what older women actually run short of, plus a few absorption realities that change around this decade of life.
The CDC’s National Health and Nutrition Examination Survey gives us a fairly precise picture of where the gaps are. Women 60 and older fall below the Estimated Average Requirement for vitamin D (about 95% of the cohort), magnesium (about 68%), and vitamin E (about 84%), and a meaningful slice are short on B12 absorption capacity even when their intake looks adequate on paper. As registered dietitian Samantha DeVito, M.S., RD, CDN, puts it: “When people think of vitamin D, they often think of bones, but it’s just as important for muscles. That means better balance, increased strength and fewer falls, which is critical as we age.”
The same eight items keep coming up across very different patients with adults 60 and up. Not because they’re trendy. Because they map to documented absorption shifts and dietary gaps that show up in this age group with surprising consistency.
The framework, in one line: Address absorption first (B12, D3, K2 with fat), close the documented dietary gaps next (magnesium, omega-3), and only then add the supplements that depend on individual lab work (calcium, CoQ10 if you’re on a statin, probiotics if your gut has been disrupted).
How This List Was Built
Three filters. First, the supplement has to address a deficit that NHANES or a comparable population dataset confirms exists in women 60+. Second, the trial evidence has to show benefit at the doses actually sold, not at pharmacological doses you’d never reach over the counter. Third, the safety profile has to be reasonable for someone who’s likely already on at least one prescription medication — drug interactions are a real consideration at this stage of life, and a supplement that creates them is a supplement that costs more than it gives.
Anything that didn’t clear all three filters is off the list. Collagen, biotin, ashwagandha, turmeric — all reasonable for specific situations, none broadly indicated for women in their 60s as a category. This list is the broad-indication eight.
Supplement Forms That Actually Matter at This Age
Before getting into the eight, a quick reference table on form factor — because for most of the supplements on this list, the form chosen at the shelf does more for absorption than the dose chosen at home.

1. Vitamin D3 (Cholecalciferol)
If only one supplement makes the cut, it’s this one. About 95% of women 60 and over fall below the Estimated Average Requirement for vitamin D. Registered dietitian Avery Zenker, RD, lays out why that matters so much for this group:
“Vitamin D helps your body absorb and use calcium to keep bones strong. When levels are too low for too long, bones can weaken, increasing the risk of osteoporosis. Osteoporosis is more common in postmenopausal women than in any other population, but adequate vitamin D intake can help reduce your risk.”
There’s a hormonal layer to it too. “After 60, estrogen declines and the body makes less vitamin D, creating a ‘double hit’ to hormone balance,” says registered dietitian Patricia Bannan, M.S., RDN — which is part of why food-only intake rarely hits the threshold linked to bone, immune, and muscle outcomes at this age.
The dose most clinical guidelines now point to for older women is 1,000–2,000 IU per day of D3 (not D2), taken with the fattiest meal of the day to drive absorption. Some women with documented deficiency need 4,000 IU under physician supervision. Above 4,000 IU without lab monitoring is a place to be careful — vitamin D toxicity is rare but real. Pair it with K2 (further down this list) and the fat-soluble math improves further.

2. Vitamin B12 (Methylcobalamin)
This is the supplement on this list that I most often see overlooked. The reason isn’t that women in their 60s eat less B12. It’s that around this decade, gastric acid production drops, and B12 absorption depends on that acid to liberate B12 from food protein. According to geriatrician Dr. Lindsey Yourman, MD, of UC San Diego, older adults can be especially susceptible to B12 deficiency because aging often runs concurrently with decreased absorption of the vitamin. Add the proton-pump inhibitors many older women take for reflux, and absorption can drop dramatically even at normal intake.
The stakes aren’t subtle. “If left untreated, vitamin B12 deficiency can lead to anemia, nerve damage and more,” says Anna Taylor, RD, LD, a registered dietitian at the Cleveland Clinic — and the catch is that blood B12 can sit at the low end of “normal” while functional markers (methylmalonic acid, homocysteine) are elevated. That gap matters because B12 deficiency at this stage can present as memory complaints, balance problems, and fatigue that get written off as “just aging.”
Methylcobalamin (the activated form) is the version I recommend for women 60+, because it skips the conversion step their bodies may not do efficiently anymore. Sublingual or oral both work — the dose, not the route, is what matters. 500–1,000 mcg daily is reasonable for ongoing supplementation.

3. Magnesium (Glycinate or Citrate)
Two-thirds of women 60+ fall below the Estimated Average Requirement for magnesium according to NHANES, and the deficit shows up clinically as muscle cramps, sleep that won’t consolidate, constipation, and blood pressure that trends up. UCLA senior dietitian Yasi Ansari, MS, RDN, makes the case that the baseline number undersells what older women actually need:
“The RDA is what’s recommended to help prevent a deficiency, but it fails to consider how much is needed for targeted needs like sleep, relaxation, digestion, muscle pain, migraines and more.”
Form is the other half of the equation. Most magnesium on shelves uses oxide, which absorbs poorly and loosens stools at any meaningful dose. “The best type of magnesium depends on your goal and your tolerance, which is why individual guidance is supportive, and working with a registered dietitian or your physician,” Ansari adds. Glycinate is gentler on the gut and absorbs well; citrate is similar but trends slightly more laxative. Aim for 200–400 mg of elemental magnesium daily — read the label carefully, because “1,000 mg magnesium glycinate” might only deliver 100 mg of elemental magnesium. If kidney function is reduced, dosing needs physician input.

A clinical note on absorptionThe pattern I see most often in 60-something patients isn’t intake — it’s absorption. PPIs for reflux, metformin for blood sugar, and reduced gastric acid all change how well several B vitamins and minerals get into circulation. If you’re on either of those medications, this list shifts: B12 becomes essential, not optional, and food sources of these nutrients matter less than you’d hope.
4. Omega-3 EPA + DHA
The argument for omega-3 in women 60+ rests on cardiovascular outcomes (REDUCE-IT, VITAL), cognitive maintenance, and the inflammatory baseline that tends to creep up with age. Cardiologist Dr. Sean R. Mendez, MD, of NewYork-Presbyterian, keeps the headline claim narrow and defensible: “The strongest evidence we have is that omega-3s lower triglycerides.”
How you take it changes how much you get. As Dr. Mendez explains:
“You also want to take it with fat. Your body needs enzymes to break down these fatty acids, which are released when you eat fat. If you take the pill on an empty stomach, you won’t absorb it as well. So, take it with a meal.”
The benchmark dose is 1,000–2,000 mg of combined EPA + DHA daily — not 2,000 mg of “fish oil,” which refers to total oil including other fats. Read the supplement facts panel for the actual EPA and DHA totals. Source matters here more than for most supplements: oxidized fish oil is worse than no fish oil, so look for third-party tested products with IFOS or USP verification. Algae-based omega-3 (DHA-dominant) is a reasonable plant alternative if fish is off the table.

5. Calcium — Yes, but With Caveats
Calcium is a supplement I recommend in a more conditional way than the prior four. The need is real — Cleveland Clinic menopause specialist Dr. Pelin Batur, MD, explains why this decade is the pressure point:
“In older adults, especially those who are post-menopausal, bone breaks down at a faster rate than it’s built. If your calcium intake is too low, this can contribute to osteoporosis.”
But more isn’t better, and the supplement isn’t the first move. “Calcium is best absorbed through the foods we eat and the beverages we drink,” says Cleveland Clinic rheumatologist Dr. Chad Deal, MD. “For most healthy people, it’s important to eat a well-balanced diet rather than relying on supplements.” High-dose calcium supplementation has been linked in some studies to cardiovascular concerns when total intake gets too high.
The current best practice: aim for total intake (food plus supplement) of about 1,200 mg daily for women 51+, and calculate your food first. If you’re eating two servings of dairy or fortified alternative plus dark leafy greens, you may need only 300–500 mg supplemental — not the 1,000 mg most bottles assume. Timing matters too: “We can typically only absorb about 500 milligrams of calcium at one time,” Dr. Batur notes, so split the dose. Use citrate (more reliably absorbed than carbonate if gastric acid is reduced), and separate it from levothyroxine by at least four hours. For most women here, the right calcium supplement isn’t 1,000 mg — it’s 400–500 mg of citrate paired with vitamin D and K2.
6. Vitamin K2 (MK-7)
This one is less widely understood than D3 and calcium, and that’s a problem because it’s what tells the calcium where to go. Cleveland Clinic dietitian Julia Zumpano, RD, LD, reframes the whole bone-health conversation around it:
“Having low levels of vitamin K is associated with a higher risk of bone fractures. We’ve always put so much emphasis on calcium for bone health. But in reality, vitamin D, vitamin K and calcium all actually work together.”
The cardiovascular half is the part most people miss. “Vitamin K has been shown to help activate a protein that helps prevent calcium from depositing in your arteries,” Zumpano explains. “Calcium deposits contribute to the development of plaque, so vitamin K does a lot of good for your heart health.” Skip K2 and the calcium-cardiovascular concern from the prior section becomes more relevant; include it and the math is meaningfully better.
The form that matters: MK-7 (menaquinone-7), not K1 — it has the longer half-life and the bone and cardiovascular trial data. The dose most studies have used is 90–180 mcg daily. One important caveat: if you take warfarin (Coumadin), K2 interacts. This is not a supplement to add without telling your prescriber.

7. CoQ10 — Mainly If You’re on a Statin
This one is conditional, and the evidence is genuinely mixed — so it’s worth being precise about what it can and can’t do. CoQ10 is made by the body, and production declines with age. The popular theory is that statins deplete it, but the cardiologists who study this aren’t sold. As Cleveland Clinic’s Dr. Leslie Cho, MD, puts it:
“Some studies show that replacing CoQ10 with a supplement could reduce the risk of muscle damage when you’re on a statin, but most studies have shown that is not the case.”
Registered dietitian Devon Peart, RD, MHSc, draws the same line on routine use: “If you don’t have a deficiency in the first place, then supplementing probably isn’t going to make any difference.” So the honest version: if you’re on a statin — and statin muscle aches are most likely in women over 65 — 100–200 mg of CoQ10 (ubiquinol, the form older adults absorb best) is a reasonable, low-risk thing to try, with the understanding that the data are split. If you’re not on a statin and have no specific cardiovascular indication, this one is optional.

8. A Multi-Strain Probiotic
Probiotics are the most variable supplement on this list because the evidence is strain-specific, not “probiotic” as a category. Gastroenterologist Dr. Trisha Pasricha, MD, is blunt about the marketing gap:
“Probiotics are a multibillion-dollar industry, but for many people, they’re not the magic fix they’re marketed to be.”
For women 60+, the genuinely useful situations are narrow: after a course of antibiotics, during travel where food and water exposure is changing, or for documented IBS-D or IBS-M (specific strains: Bifidobacterium infantis 35624 for IBS, Lactobacillus rhamnosus GG for antibiotic-associated diarrhea). I don’t recommend a daily probiotic indefinitely for every woman in this age group — I recommend a targeted, strain-specific probiotic for specific situations, and a food-first approach the rest of the time. Pasricha’s prescription is the same: “The more diverse your plant intake in your diet—fruits, vegetables, legumes, nuts, whole grains—the more resilient your microbiome becomes. Supplements don’t substitute for that foundation.” The fiber side of the protein-vs-fiber conversation deserves more attention than it usually gets in this cohort.
If you do supplement, look for products that list specific strains (not just genus and species), refrigerate-required formulations, and at least 10 billion CFU at expiration date — not at manufacture.
Three Conditions That Change This List Entirely
This is the part most articles skip. The eight supplements above are the baseline for a generally healthy woman 60+. Three conditions change the list materially:
Chronic kidney disease. Magnesium dosing needs physician input. Calcium and vitamin D need monitoring. Phosphorus restrictions may make some supplements off-limits. This list is not appropriate without nephrology input.
Anticoagulant therapy (warfarin, apixaban, rivaroxaban). Vitamin K2 needs to be discussed with the prescribing physician. Omega-3 at high doses can affect bleeding time. Ginkgo, garlic, and high-dose vitamin E (not on this list, but commonly added) compound the effect.
Thyroid disease on replacement therapy. Calcium and iron supplements interfere with levothyroxine absorption — separate by four hours. Selenium is sometimes added but should be discussed with the prescribing endocrinologist first.
What Labs to Ask Your Doctor For
Before adding most of these, the most useful baseline labs are: 25-hydroxyvitamin D, vitamin B12 with methylmalonic acid (the functional B12 marker that catches deficiency when serum looks normal), a CBC and a basic metabolic panel, ferritin if fatigue is the chief complaint, and TSH. If you’re on a statin and considering CoQ10, no specific lab is needed — the indication is symptomatic.
A useful rule of thumb: if a supplement is on this list and you’ve never had the corresponding lab drawn, the lab is the first move, not the supplement.
A Decision Tree to Work Through
If I had to compress this list into a five-question filter for a woman in her 60s who’s never built out a supplement routine, this is what I’d use:
1. Do you take a PPI (omeprazole, pantoprazole) or metformin? Add B12 (methylcobalamin), 500–1,000 mcg daily. Non-negotiable.
2. Have you had a vitamin D level drawn in the last two years? If no — get one. If yes and you’re below 30 ng/mL — D3 at 1,000–2,000 IU daily with K2.
3. Do you eat fish two or more times per week? If no — omega-3 (1,000 mg EPA+DHA combined). If yes — optional.
4. Do you cramp at night, sleep poorly, or have constipation you can’t otherwise explain? Magnesium glycinate 200–400 mg elemental at bedtime.
5. Are you on a statin? CoQ10 (ubiquinol) 100–200 mg daily.
That’s it. Probiotics and calcium move from the always-list to the it-depends-list. Everything else is downstream of those five questions. If you’ve been spending more than $50 a month on a multivitamin stack and you can’t answer those five, the stack is probably bigger than it needs to be — and missing one or two things that matter more than what’s in it.
Note on form factors: gummy vitamins consistently under-deliver on labeled dose; in third-party testing, multiple brands have come in 30–60% below label. For the supplements on this list, capsules or tablets from third-party-tested brands are the safer call.
